PATIENT INFORMATION

First Visit

If you have a painful tooth and are fairly healthy, we can often treat you at your first visit. However, your initial appointment may instead consist of a consultation explaining your diagnosis and treatment options. If you have a complex medical history or an involved plan of care, we may need an initial evaluation. A second appointment would then be scheduled for you to provide carefully planned treatment.

Please assist us to best use your time by providing the following information at the time of your Consultation visit:

  • Your surgical referral slip from your dentist and any x-rays, if you were given these.
  • A complete list of medications you are presently taking, along with any drug allergies or sensitivities. If you already have a handwritten, or computer-generated list of your medical problems and medications, please bring it along. (Click here to download our Patient Health History & Registration Form).
  • If you have medical or dental insurance, please bring all necessary insurance cards and information. This will save you time and allow us to help process any claims more rapidly for you. (Click here to download our Insurance Information Form).
  • Any downloaded forms may be completed at home to save you time. Please bring them to your first visit.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian AT ALL VISITS. If you are over 18 and financially responsible for your care, a parent may still be helpful at your first appointment to help you with your decisions.

A pre-operative consultation and an exam are done routinely for patients who desire general anesthesia or IV sedation for their surgery. This is the same safety protocol followed in most hospitals.

Please make sure that you have a complete list of your medications so that we may best care for you.

X-Ray

If your dentist or physician has taken x-rays, you should request that they mail or e-mail them to our office before your day of appointment. If there is not enough time for mailing before we see you, please pick up your x-rays yourself and hand-carry them to our office. If additional films are necessary, they can be taken at our facility.
