If you have a painful tooth and are fairly healthy, we can often treat you on the day you first visit us, if you wish. However your initial appointment may consist of a consultation explaining your diagnosis and treatment options. A complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.
Please assist us to best utilize your time by providing the following information at the time of your consultation:
- Your surgical referral slip from your dentist and any X-rays if applicable

- If you have medical or dental insurance, bring the necessary insurance cards or information. This will save you time and allow us to help you process any claims. (Click here to download our Insurance Information Form)
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at all visits. If you are over 18 and financially responsible for your care, he/she should also come for this appointment.
A pre-operative consultation and an exam for your well-being is done for patients who wish IV anesthesia or sedation for surgery, as would be done in a hospital.
Please alert the office if you have a medical condition that may be of concern, prior to surgery (diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (heart medications, aspirin, anticoagulant therapy, etc.)
X-Ray
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility. We want to avoid your extra exposure to films, unless truly needed..
